2020 Gold Rating from the National Association of Free and Charitable Clinics (NAFC) Quality Standards Program

MHMC proudly announces the earning of the 2020 Gold Rating from the National Association of Free and Charitable Clinics (NAFC) Quality Standards Program. Our seal allows us to showcase the quality of care provided at the national level.
Note that NAFC’s mission ensures the medically underserved have access to affordable health care. NAFC and its members dedicate to ensuring that patients receive quality health care. Therefore, to quantify and qualify the care provided at the Free and Charitable Clinic network, the NAFC formalized a set of Quality Standards for member organizations.
These standards allow NAFC to showcase the quality care provided to patients, to policy makers, partners, funders and stakeholders. Additionally, these standards assist the NAFC in developing benefits and resources that will help members enhance the care to patients.
Our 2020 Annual Member Data & Standards Survey earned us the NAFC 2020 Standards Gold Rating that showcases the quality of care we provide at the national level by NAFC!
NAFC Quality Standards elements include policies and procedures related to the following areas: Administrative, Enhanced Access and Continuity of Care, Identifying and Managing Patient Population information, Planning and Managing Care, Providing Self-Care Support and Community Resources, Tracking and Coordinating Care, and Measuring and Improving Performance.
